evaluation survey postcards
Has/does anyone give out surveys for customers to evaluate the service that you give them?
I think that I am going to try it.
What do you think would be good questions?
Maybe my customers can give me some insight for improvement from their eyes.

I implemented a similar survey and it was very successful for us.
The questions I asked on the back of a stamped postcard I gave the customers to return to us were:
1) Were you happy with our work?
2) Would you use our service again?
3) Would you refer us?
4) Additional Comments:_________________________________

Add number 5 as well, "What could we have done better".
I am a firm believer that you learn more from negative feedback than by positive and so I would provide the customer a chance to be more critical and developed in their answers.

List some categories and have them rate you from A to F

Knowledge

I used to have my technicians hand out a survey and self addressed envelope with the invoice. Got a good return, learned some things, and was able to use the comments on my web-site. With the cost of postage now, I just put a "note from the owner" on the bottom of the invoice. It basically says thank you, if you have a problem please call me immediately, If you are happy with our service feel free to let me know but definitely let a friend know.
